Weight loss trials make headlines, but hidden behind the big claims are subtle design issues that could weaken the findings. Experts recommend readers ask five key questions before believing a diet study headline. Let's look at each in turn to understand how to judge the impact of a trial.
Why the headline can mislead
In one study, 155 people were randomized to explore a diet and exercise intervention, but only after 746 telephone inquiries. Another trial excluded 12% of consented participants, largely for medical reasons, before progressing to randomization. How many participants remain after these filters matters, and their path to the trial speaks to the study's representativeness.
Not all recruits can go on to maintenance or reach the final analysis. In a 2014 BMJ systematic review and meta-analysis, 7,788 out of the original 9,451 initial recruits made it to the maintenance phase. Between that phase and the end of the treatment, a further 20% dropped out on average. To put it another way, only 65% of initial recruits made it to the completion of the treatment in this review.
The dropout matters: when a third or more of the participants stop participating early, the reasons may be related to the intervention. Thus, their inclusion could weaken a trial's claims about ease and effectiveness.
Sample size and how much trial survived
Of course, diet trials are not all designed for the most challenging participants. But when they are, you should watch the numbers, especially in the weeks that follow. In studies with very strict adherence and high dropout, as little as 65% of initially recruits remained to the final analysis, as we noted above.
But it is not only the proportion of recruited, randomized, and maintained participants that matters to interpreting study headlines. You must also know exactly how many were randomized and analyzed, and understand how study design strengthened or weakened the result. Outcomes are most certain across a large, randomized study with few excluded or lost to attrition, but smaller and less adhered-to studies can still yield important results, especially about the efficacy and adherence of an intervention regimen.
In one weight-loss maintenance trial, participants were randomized to an intervention group after the weight-loss phase. If an attrition rate in either arm of the randomized trial averages over 20%, the benefit or effect or risk elicited may not be attributable to the intervention alone. On the other hand, lost-to-follow-up is not per se a grounds for invalidating findings, which depend on the size-of-treatment effect, the comparison arm's correlation with the experimental arm, and the population from which the sample was drawn.
Duration and dropout
When 29 out of 119 participants dropped out of a 7-month weight-loss phase, that average 24% dropout rate lowered the potential scale of findings' impact. On the other hand, a low attrition rate does not mean a study will stand scrutiny. How the non-completers' outcomes correlate with the completers can undermine a trial's final claims.
That relationship depends on why the participants dropped out. In some cases, the attrition is due to success in adherence, not failure; the participants lost their target weight in the maintenance phase. In other cases, attrition means that the maintenance regimen was not held by the cohort.
For this reason, quality ratings of diet studies keep close track of the dropout rate, but the rate alone does not tell the whole story. In a table that tracks short-term physical-activity interventions for weight loss, a CDC-affiliated 2024 review used a study-quality item that asks whether the overall dropout rate remains at 20% or less. But those also reporting an average dropout over 20% are kept in the review, alongside other quality markers and outcomes. You must watch for dropout, but can trust studies with higher attrition when they come with clear explanations.
Comparison group and what was actually compared
Only by identifying the study's controls or comparison cohort can a reader decide if the findings weigh on an intervention. If the comparison arm passes the 20% dropout rate criterion, for example, that tells the reader something about how the other arm likely fares in comparison. If randomized, the comparison group may answer key questions about what the experimental arm participants avoided merely by their randomized group affiliation.
For this reason, the 2024 CDC synthesis table of physical-activity trials explicitly tracks the sample size, intervention group, control group, and intervention duration and intensity. The trials chart their duration, too, so you know how long a regimen was held before a weight-loss outcome was measured. But crucially, these measures and outcomes are not reported in isolation; they are arrayed along with comparison groups, funding sources, and other quality metrics.
This comparison arm allows you to assess the intervention's impact without simply assuming causation. Even a modest effect may be statistically significant in an otherwise high-fidelity trial, but a control group helps you understand the intervention's relative efficacy, relative adherence, and relative side effects.
Funding and what it does not mean
Comparing groups and watching attrition help track a study's efficacy and fidelity, but funding can only amplify or quash a reader's trust in the rendition. Funding is never a cause for outright discounting a trial; disclosure is the key. Look for the funding statement, whether in the abstract or cited alongside the method.
Funding statements accompany both non-directional probes and drug-focused trials. Industry-founded studies are only one indication. It is the disclosure itself that legitimizes a trial, not the source of funding. Indeed, The BMJ review found that 14 studies disclosed industry funding.
Industry funding provides information about the study's rigor, but does not mean the study is unsound. Readers instead need the five-check method we've been through: note sample size and duration, compare the dropout rate to the 20% threshold, and consider the clarity of the comparison arm. With this method, you can meet the next headline with confidence.
The reusable checklist
Now, when headlines promise a weight loss solution, remember these questions:
- How many participants started the trial, and how many were still in it at the end?
- What was the follow-up duration, and how did the study define dropout?
- What were the characteristics of the comparison group, and how did they differ from the experimental group in attrition and adherence?
- What funding supported the trial, and why does that matter for understanding its limitations and rigor?
- Is the comparator group even provided or is the study an uncontrolled looking at each participant against themself?
With this checklist, you can evaluate any diet study's claims, regardless of their source. But watch for the five checks in the one place the experts publish them.
